MEMBERSHIP FORM TO JOIN THE OFFICIAL VINCENT IRIZARRY FAN CLUB

Anyone* who appreciates the artistic work of Vincent Irizarry and supports the concept of creating a caring and charitable community of friends can join Friends Of Vincent Irizarry. At this time, FOVI is operating dues-free. All you need is a valid e-mail address, since FOVI operates on-line.

Fill in the form below and submit your request for membership in FOVI. All information is strictly confidential and will be used only to confirm your membership and to send you special FOVI mailings.

Once your request has been processed, we will e-mail you a Welcome Letter confirming your membership and a copy of FOVI's Guidelines.

I have read and understand the goals and purpose of FOVI, and I want to be a member. Submission of my online application membership form signifies that I have read, agreed, and accepted FOVI's Terms of Service, which contains important disclaimers and legal information.

* If you are a minor, please submit a written statement indicating your age and have it signed by a parent or legal guardian which consents to your membership to Friends of Vincent Irizarry. Please mail the consent statement to: Friends of Vincent Irizarry, P.O. Box 129, Warrenville, IL 60555. Once written consent is received, your online application form will be processed. Submittal of required written consent or any representation of the age of majority will be accepted as true, and the applicant (along with attendant family members/guardians) bears all responsibility or subsequent liability resulting from any submitted representation, true or falsified. In accordance with the Children's Online Privacy Protection Act, any data, except e-mail addresses, that FOVI collects on children under 13 years of age will be discarded. E-mail addresses of children under 13 submitted during registration are kept by FOVI for identification purposes only, and under no circumstances, are displayed or made publicly available.
